I understand by signing this form I am assuming all responsibilities of the mascot
uniform including but not limited to storage, use, cleanliness, and return in its
original condition. I understand that I will be held responsible for any and all damages
to the uniform or any cleaning costs associated with its use. Furthermore I understand
that the uniform should not be used outside of what it has been requested and approved
for. Anyone wearing the uniform must follow the University Code of Conduct and understand
that they are representing the University both on and off campus. Anyone caught misusing
the uniform will be reported to the Student Conduct Office, University Police, or
other authorities.
