Registration for the PEBTF’s Know Your Numbers Wellness Screening Program opened on July 1, 2021.

Deadline to complete the Get Healthy Know Your Numbers Wellness Screening is December 31, 2021, which qualifies participants for the lower health care contribution rate for July 1, 2022 through June 30, 2023.

The PEBTF has decided to have very limited onsite events this year due to on-going COVID-19 guidelines.  As a result, there will not be a screening event at Cal U.

 

There are two way to obtain your screening:

• Quest Diagnostics^® Patient Service Center.
• Your doctor’s office.

 

To register for a screening:

• Go to www.pebtf.org
• Click on the Get Healthy logo on the right and follow the instructions.

 

The Wellness Screening:

• Includes a blood test for cholesterol and blood glucose (sugar) levels and height, weight and blood pressure measurements.
• Is free at an onsite event or a Quest Patient Service Center (you may also obtain the screening at your doctor’s office but you will have to pay a copay).
• Is fast – takes about 15 minutes.
• Is easy to schedule – online at www.pebtf.org (Click on the Get Healthy logo and continue to the Quest Diagnostics Site) or by phone at 855-623-9355 to register Monday – Friday, 8:00 a.m. to 8:00 p.m. and Saturday 8:30 a.m. to 3:00 p.m.
• Spouses/Domestic Partners are not required to participate this year.

 

If you choose to visit your physician, the Physician Results Form must be completed and faxed to Quest Diagnostics by December 31, 2021 in order for you to qualify as completing the wellness screening.  You must obtain the Physician Results Form through the PEBTF Get Healthy site.  The form will be populated with your information (not your social security number) and will include a barcode allowing for accuracy.
